ATLAS Performance Complex Consent & Participation Waiver of Liability (minor)

All participants and parents must agree to this Consent Form & Liability Waiver for their child to be eligible to participate in ATLAS Performance Complex training services.

I hereby grant permission for myself/my child to participate in training services at ATLAS Performance Complex. I understand and acknowledge that I/my child may incur personal or bodily harm while participating in sports-related activities, including but not limited to activities described as running, jumping, landing, strength and conditioning, etc. By agreeing to this consent and contract participation waiver of liability, I assume all risks inherent in these activities and accept full responsibility for all damages or injuries of any kind; but I am also aware that reasonable care and supervision will be exercised by ATLAS Performance Complex staff to provide for the general well-being of myself/my child. I further acknowledge that I am placing my trust in myself/my child to adhere to proper standards of conduct and follow the rules set forth by ATLAS Performance Complex, or risk removal from the building and program.

I understand and assume the risks described above and those inherent with such activities, and I individually, on behalf of myself/my child, do hereby release and discharge and covenant not to sue ATLAS Performance Complex, its officers, directors, employees, agents and/or volunteers for this activity and agree to hold them harmless from any and all claims whatsoever arising from the conduct of any person(s) which result in the injury or loss to my child or myself. I accept responsibility of notifying ATLAS Performance Complex's staff of any pre-existing conditions affecting myself/my child. All participants must have health insurance while participating in ATLAS Perofrmance Complex training services.

I hereby grant permission for images of myself/my child, captured during ATLAS Performance Complex's training through video, photo, and digital camera, to be used solely for the purposes of ATLAS Performance Complex's promotional material and publications, and waive any rights of compensation or ownership thereto.
